By filling out this form and submitting it, you are agreeing to have a THE NANNY representative contact you for your credit card information so that a 4-hour minimum can be charged to your credit card.

Your nanny is not confirmed until a representative notifies you! We will contact you as soon as possible.

You must give THE NANNY a two week notice before leaving the program.

I/We claim that all information above is factual and correct and understand that sensitive contents will be treated as confidential and not shared with anyone unless in the course of medical or legal action.
